Thank you for choosing Dr. Lamb and his staff for your oral care! Our most essential office policies and practices are explained below. Please contact us with any questions or concerns.
Payment
Payment is expected at the time the service is provided. You can make your payment in cash, check, credit card, or debit card. We accept Visa, MasterCard, Discover, and American Express.

Insurance​
We will charge your insurance company as a courtesy if you supply us with the necessary paperwork. Copayments and deductibles must be paid in full at the time of service. Your agreement with the insurance carrier is between you and your insurance provider, and as such, we do not routinely investigate refused claims or claims that have not been paid in accordance with the contract. You will be liable for the unpaid balance if your insurance provider does not pay within 60 days of the date on the invoice. Any services performed that are not covered by your insurance plan must be paid in full at the time of your visit or when you get notice of a denied insurance claim.

Late Payment
​Unless prior agreements have been made, a 2% service fee may be charged to any outstanding debt that is over 60 days old.

Delinquent Accounts
​If an account is not paid as requested, it will be turned over to a collection agency, and a 30% charge will be applied to the amount. This payment must be paid in full before any additional appointments may be booked.

Appointments
As a courtesy to other patients and employees, please plan to be on time for your appointment. Call us today to schedule your appointment.

Cancellations
Please be respectful of other patients and the doctor by providing a 24-hour notice if you plan to cancel your appointment. If you do not submit a 24-hour notice during the week, your account will be charged a minimum $50 "No Show or Late Cancel" fee.

Medication
Please bring your whole prescription (or a list of medications) to each session. The doctor will review allergies, instructions, side effects, prescription interactions, and doses.

New Patient Process
​Please come 10 minutes early for your initial appointment. You should bring the following items:

Insurance card

Picture ID, such as Driver’s License

All your medications (or a list), including any vitamins or supplements you take regularly

Name, phone number, and fax number of your previous physician
