New Patient Process

Please come 10 minutes early for your initial appointment visit. You should bring the following items:

• Completed New Patient Forms (click on links below to print)
• Insurance card
• Picture ID, such as Driver’s License
• All your medications (or a list), including any vitamins or supplements you take regularly
• Name, phone number, and fax number of your previous physician
• Immunization records

Payment is due at the time of service. We accept cash, credit card, and debit card payments. Checks and American Express are not accepted. Contact our office for more information.
